Dana Scully's ID card

What does her ID card look like exactly? I'm going as her to a fancy dress thing. I've got flashlight, briefcase and gun (fake of course) but I need to get the card right

This is probably too late and the event has probably already passed... but... just in case, this is what I have. This is the replica which is sold. I'm not sure if it's identical to the one she has on the show, but it should be close enough.
