Fantasy Maps

I tried to search for something like this but apparently the word "map" is too small and common for the function to find any thread relevance. Please someone redirect me if they know of a thread with this topic.

Some genres (usually fantasy of course) have maps printed in the books that readers can refer to when they hear of a place in the book. Everyone knows this of course but I have been having trouble making maps or sketches on programs that exist on my computer and I'm sure professionals don't do it by hand.. right?

So I was wondering if anyone had any tips on what to use or how to go about creating maps for you books. Even a link to some software or something would be useful.

I love the way pTerry did it, though. For years he left map-creation to the reader, then created two which made the bestseller lists.
