80's book about young man with a curse following him

Read a book mid to late 80's a young man warrior or wizard pursued by a sentient curse plague. Curse portrayed as some kind horse. Thought it was a Hugh cook novel.

Could it be Richard A Knaak's "Shadowsteed" or another book in the Dragon Realm series, e.g. "Firedrake"? The horse is called "Dark horse".

I wonder did the young man have stars on his forehead? a trilogy? Riddle Master of Heed? He becomes a warrior/wizard and is pursued..
