A Novel about bugs or insects ruling a world.

My wife said she read a book about 30 years or so ago (late 80's Early 90's) that was about a person who went to sleep and woke up in a world ruled by insects or bugs of some kind. The main character wakes up on a beach but thought they were in a hospital. Then the main character said to themselves something like “if I was in a hospital why am I on a beach and why do I have a sword?” The humans of this world had to pay tribute to the bugs. They would hang the humans on the wall of the city. The bug queen had sex with a human man and kept making references to humans as batteries. This was not a series.
