Looking for sci-fi story from 90's (mid-90's I think)

Looking for a book that was written in the 90's featuring a young man (teenager? )who along with a few people from a scouting counsel travel to the planet his ancestors were from. He grew up on a planet full of telepathic empathic aliens. When he was young he was involved in an accident that damages his brain and so a device is implanted in order to help keep him alive. The planet his scouting group traveled to via spaceship (his ancestor's native planet) is ravaged by the changing climates and the remaining survivors of the planet live primitive lives full of hardship. The MC infiltrates the primitive outpost and at the end unites his group of scouting scientists and the survivors who ancestors hadn't left the planet.
