Basically, the book is about a changing culture. Someone finds a "cure" for AIDS (this was back when AIDS was a death sentence), but the "cure" has an unintended consequence. It screws-up the DNA of all future-born children, such that after puberty they switch biological sex periodically throughout their lives.
