Planning scene scene plan

When things don’t go as intended sometimes your characters need to regroup and make a new plan of action. What do you feel are the tension drivers in such scenes? Just the desire to know what they will attempt, or are there other things you think belong in such scenes? Maybe be a disagreement about how to proceed? Some external pressure that makes the planning difficult?
What have you done in the past or have seen done well?
 
Well, I have those scenes quite often in my story. I like to focus on how the characters react when things don't go as planned. In that sense, many old and often unrelated conflicts arise just because the characters are stressed out by frustration and confusion.
