watched it tonight too... I also noticed that but if you noticed when willow entered the room all electronics went goofy, maybe that's why the heart monitor flatlined...(sorry just know my medicine a bit)

Interesting point though, merits further discussion

yeah i just rewatched this today, and thought the same thing.

But if that wasn't it - both Prophecy Girl and the Gift and Once More with Feeling are before this ep so they wouldn't be able to include her third death. I can't remember if anyone states she's died twice after this episode. (probably though as they seem to like it :D)

Generally, to actually declare someone dead though the heart and breathing (and neural activity) have to have been stopped for 2 minutes so would they split second that she flatlined even count?

In the S7 ep "Dirty Girls", Xander says that "This girl has died, two times, and she's still standing". But on quizzes and things, that's always the answer too.

I'm not gonna pretend to know anything about medicine (except what I've seen on Casualty and ER :p). So maybe it doesn't count because it was so quick. But, to be fair, we don't know how long she was out in "Prophecy Girl" before Xander and Angel found her.

Steffi has a good point too - maybe it was just Willow's magic that caused the flatline, rather than Buffy's heart stopping.
