Yep, that sounds about right to me. It was the Sao Paolo (Defiant-A), which was destroyed by the Breen, then the Defiant again.

If my knowledge is correct Vedek Farla Kiras old mentor, who has since joined the cult of the Pah Wraiths, kidnaps Kira Neris via transporter and delivers her to Gul Dukat on Empok Nor, who wants to iniciate her into the cult.
There Kira exposes Dukats liason with a follower which resulted in a birth of a half Bajoran half Cardassian baby and she also exposes Dukats plan for a mass suicide for his followers. After learning this her old mentor Vedek Farla commits suicide.
