Or too much imagination. Lieutenant comes from the French, lieu tenant, which means "in place of the tenant". The tenant was the landlord and the lieu tenant was his chief assistant.
 
In the TOS, there are no enlisted serving aboard ship. It was not until Chief O'Brien showed up and any enlisted rank appeared in Star Trek.
Apart from the Yeomen! It always seemed to me to be quite a large step backward from 'The Cage' where "No. 1" was a female command officer, to the second pilot episode where all the females were in miniskirts and had secretarial jobs. All Uhura ever did was "open hailing frequencies", (and kiss the Captain.) All Rand ever did was ask for a signature in a soft focus lens.

But as I said earlier, rather than copy other writers mistakes, design your own system based upon the great Naval history that exists already.
