Usually when posters refer to the two least favorite deaths in AGOT, they are referring to humans. Eddard for sure... then usually Drogo... or even Syrio.
Lady's death may not have been the most overtly formative occurrence in Sansa's life, but it may have been the most momentous (after Eddard's death). Lady's death sundered whatever relationship Sansa and Arya had. It also proved to Sansa that her father could make mistakes... which, probably in large part, drove her to confess Eddard's plans to Cersei. Lady never would have survived Cersei's and Janos' purge, but her presence up to that point may have kept Sansa from needing reassurances from Joffrey and Cersei.
