I had an autocorrect issue once in a works email that changed 'Apologies for any inconvenience caused' to 'Apologies for any incontinence caused'.

Fortunately the member of the public who received the email had a sense of humour.
The worst one I have encountered was in an airshow's brochure. They attempted to announce that there would be a PUBLIC display on the tarmac...

A colleague was nearly put on a disciplinary for the same sort of thing. The union had it over turned, mind you I supposed it helped that the letter inviting him too a meeting to discuss his actions was full of typos including the line

formal disability procedures will be considered.

All letters and emails were standardised after this, any variations had to be proof read.

Then the time a lady phoned a bereavement line to say she had lost her husband, "I'm really sorry , hope you find him soon"
 
I've name-dropped this month's theme a total of seven times in my contribution. I doubt that this will be bettered. Perhaps "bettered" is not the right choice of word.

(Yuck, Cathbad! I don't like the image that story left in my overly active imagination.)
